Right of Way Permit Application

This application shall be accompanied by one (1) copy of plans, specifications and other pertinent information

which show all lot dimensions, exact locations of buildings, set backs, right-of-ways, easements, parking and

driveway layouts. All site dimensions shall be based on actual survey of plat drawings.

PURPOSE OF PERMIT

MATERIAL BEING REMOVED

It shall be the responsibility of the applicant and/or agent to perform the work in an expeditious manner, including proper barricading and lighting to safeguard the public. All excavations and disturbed areas shall be back filled, repaved and restored in accordance with village specifications. A traffic control plan will need to be submitted. Street excavations/cuts/boring will require a restoration plan to be submitted with the application. Non-compliance with any of these requirements can be cause for forfeiture of a portion or all of bond.

PERMITS CANNOT BE PROCESSED IF DRAWINGS ARE NOT ATTACHED

The undersigned hereby applies for a zoning permit based on the information above including any attached drawings and certifies the information to be correct.
